facing Ongoing Screening (OGS) issue for few records randomly.

Hi,

We are facing the OGS issue for our production records where the screening value is not setted. nearly for 1k records we had the value as No. actually it should be setted to yes using our custom code.The same code was working fine for the rest of the records.
so can you please provide us the detail reason why the records were supposed to fail for few records.
we had this as P1 issue in our production environment.
please help us ASAP.
