This sounds like this could be new development? You would best be served if you used the REST API, since the SOAP is not longer being updated. You can download the REST API Example application from the following link, using the “Example App” menu option:

Although the Example App demonstrates the use of C#, the application provides the raw HTTP requests and responses so it is very helpful to use for any language. There is an on demand example of the Ratings template which can be accessed via the Example App menu by selecting "On Demand Extractions" and "Create Ratings" option.
If you are already using the SOAP API and need a quick solution, there is samples of using the Ratings template in the which can be accessed from the sample application "Sample" tree, by selecting "Extraction", "Other" and "Ratings Sample".
